Responsibilities
- Conduct patient evaluations and develop individualized physical therapy treatment plans for residents in a geriatric and long-term care setting.
- Implement therapeutic exercises, gait and balance training, strength programs, functional mobility activities, and appropriate modalities to improve function and independence.
- Monitor progress, document treatments, update goals, and adjust plans based on resident response and interdisciplinary input.
- Collaborate with nursing, rehabilitation, social work, and ancillary teams to coordinate care and support discharge planning as appropriate.
- Educate patients and family members on home exercise programs, safety, fall prevention, and activity modification to promote continued progress.
- Ensure safety, infection control, and quality standards are maintained; complete timely, accurate documentation and compliance with regulatory requirements.

Requirements
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Current, unrestricted physical therapy license or eligibility to practice in Montana; ability to obtain licensure as required.
- Minimum 1 year of physical therapy experience; experience in geriatric or skilled nursing/long-term care settings preferred.
- Strong documentation, communication, and collaboration skills; able to work effectively within an interdisciplinary team.
- Availability to work a daytime schedule and participate in a travel assignment as required.
- Basic CPR/BLS certification or willingness to obtain.
