Job Description

A contract opportunity is available for an experienced Nurse Practitioner to deliver high-quality care to patients residing in skilled nursing and assisted living facilities. This role is focused on providing comprehensive care for insurance members, preventing unnecessary hospitalizations, and facilitating effective transitions back to home environments. The program emphasizes regular health assessments, care coordination, and interventions that enhance patient outcomes within the long-term care setting.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• No weekends or holidays required

Key Responsibilities:

• Develop comprehensive problem lists and medical treatment plans for each resident
• Provide primary, preventive, urgent, and acute on-site care
• Identify and regularly monitor high-risk patients, increasing visit frequency as needed
• Establish contingency plans for managing high-risk clinical conditions
• Collaborate and communicate effectively with residents primary care physicians
• Deliver hands-on bedside care and create individualized care plans and medical orders in collaboration with facility staff, Directors of Nursing, and primary care providers
• Assess newly admitted patients following hospital discharge, supporting safe and effective transitions out of skilled nursing facilities
• Manage a panel of assigned members across multiple nursing facilities, typically working at one facility per day but available to assess acute cases as necessary

Qualifications:

• Master of Science in Nursing (MSN)
• Current Basic Life Support (BLS) certification
• Active Nurse Practitioner (NP) license in the state of New York

This position offers a competitive hourly rate ranging from $75 to $100, reflecting experience and expertise in the field.
