Registered Nurse (LTC) – Middlebury, Vermont
Travel assignment starting 10/22/2026 for 5 weeks
Weekly pay: $1,689 – $1,840 per week

Setting: Long-term care nursing home with a post-acute skilled nursing rehab unit

Responsibilities
- Provide direct patient care to adults 60+ in a post-acute skilled nursing rehabilitation setting
- Administer medications and perform RN duties in accordance with physician orders and facility protocols
- Monitor patient status, perform assessments, document in the electronic health record, and communicate changes to the care team
- Support care planning and collaborate with nursing, therapy, and ancillary staff to meet rehabilitation goals
- Assist with technical tasks such as phlebotomy, EKGs, and IV starts as per unit procedures
- Ensure safe patient handling, comfort, and dementia-capable care when applicable
- Participate in unit activities and respond to patient care needs, including occasional cross-unit floating

Requirements
- Active registered nurse license in Vermont (RN)
- BLS certification
- Minimum 1 year of clinical RN experience; prior travel experience preferred
- EPIC experience within the past 2 years is required
- Dementia-capable care certification preferred
- Ability to work a 12-hour night shift and to participate in occasional holiday coverage
- License must be current and on file at time of submission
- Background checks: nationwide/7-year history with FACIS-3 and related screenings within required timelines
- Must be able to meet unit-specific requirements and safety standards for post-acute LTC care
