Join our Maternal Fetal Medicine team as a Nurse Navigator! This role provides specialized care for high-risk pregnant patients with maternal conditions such as diabetes, hypertension and other complex needs. This salaried role offers a respected position within the department and a supportive team environment. The Nurse Navigator will work exclusively at our Maternal Fetal Medicine sites, collaborating closely with multidisciplinary teams to coordinate patient care. Responsibilities include performing non-stress tests (NSTs), supporting in-office procedures and ensuring seamless communication between patients, providers and care teams. This role will be responsible for providing education and support to inpatients and outpatients. Responsibilities include addressing healthcare disparities and barriers to care for patients. May participate in some aspects of clinical care.

Responsibilities include addressing healthcare disparities and barriers to care for patients; including facilitation of communication across the continuum of care, coordination of resources, education and advocacy, and coordination of care with managing physicians and the interdisciplinary team. May participate in some aspects of clinical care.

Responsibilities And Duties:

• Assesses learning needs, provides education, assesses learning achieved, and provides support and navigation to patients and families.
• Participates in quality control and assurance.
• Participates in process improvement activities.
• May participate in some aspects of clinical care as needed.
• Other duties may vary by department such as various program coordination and maintaining registries.

Minimum Qualifications:

• Bachelor's Degree (Required)
• RN - Registered Nurse - Ohio Board of Nursing

Additional Job Description:

Required:

• Ohio RN License
• Bachelor's of Nursing (BSN)
• Minimum of 3 years RN experience

Highly Desirable:

• EFM certification preferred.
• Experience with nonstress test (NST) highly desirable.
• Women's Health experience (OB office and/or L&D preferred)

Work Shift: Day

Scheduled Weekly Hours: 40
