Contract | Days | 8-Hour Shifts | Every Other Weekend | Inpatient Only

Key Responsibilities

• Provide physical therapy services primarily within a skilled nursing facility setting, with the ability to float to an inpatient acute care hospital as needed
• Treat approximately 8–9 post-acute patients per day across assigned stations (A, B, C, and D)
• Perform patient evaluations and treatments for diagnoses including total joint replacements (TKA, THA), CVA/stroke, COPD, pneumonia, deconditioning, and balance difficulties associated with general medical conditions
• Work with a geriatric population presenting with chronic, orthopedic, and neurological conditions
• Perform patient transfers independently and safely
• Collaborate closely with an interdisciplinary team and maintain excellent communication across care settings

Required Qualifications

• Active California Physical Therapist (PT) license required
• AHA BLS certification required
• Ability to work independently and perform patient transfers
• Strong communication skills and experience collaborating with interdisciplinary teams
• Proficiency or ability to learn Cerner EMR

Preferred Experience / Skills

• Minimum of 6 months of clinical experience in skilled nursing or acute care settings (new graduates with qualifying experience will be considered)
• Prior experience in a skilled nursing facility setting is preferred
• Familiarity with geriatric, orthopedic, and neurological patient populations
