Mobile Wellness Advocate (Community Health Worker)

Berger Health System

OhioHealth in Central Ohio is seeking a Community Health Worker (CHW) to work with patients identified as facing non-clinical barriers to care. The CHW will collaborate with a multidisciplinary team to identify barriers and coordinate community resources, providing individual consultation and follow-up based on social determinants of health. Responsibilities include direct patient support (in-person or by phone), home or community visits, documenting interactions, and maintaining HIPAA-compliant #J-18808-Ljbffr Berger Health System
