Case Manager - Staten Island Treatment Alternatives for Safer Communities (TASC)

EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full-time Case Manager to join the Staten Island Treatment Alternatives for Safer Communities (TASC) program. Job Details Location: 120 Stuyvesant Place Suite 410, Staten Island, NY 10301 Position Type: Full Time Education Level: Associate Degree Salary: $23.00 per hour Schedule: 9AM–5PM, Monday–Friday Travel: Up to 25% Responsibilities Act as a liaison to the courts. Conduct assessment interviews and arrange for clients to be placed into appropriate treatment programs. Investigate and arrange client service plans. Complete follow-up paperwork and maintain complete client records, making them available for inspection by supervision at any time. Prepare or obtain monthly and quarterly progress reports as required by funders. Prepare and present written status reports to the criminal justice system in a timely manner regarding client progress. Investigate and arrange supportive services for clients. Escort clients to appointments and treatment facilities when indicated. Maintain a caseload of offenders. Make monthly site visits to all treatment agencies assigned to the caseload. Conduct drug testing on program clients as needed. Conduct cognitive behavioral therapy interventions; provide HIV/Hepatitis testing resources and referrals to treatment. Qualifications Associate degree in a helping profession and at least 2 years of prior experience working in intervention programs with criminal justice-involved participants. Bachelor’s degree preferred. Experience conducting health education groups and/or cognitive-behavioral interventions preferred. Knowledge of treatment programs and local criminal justice and health systems preferred. Good communication skills, both oral and written; ability to work cooperatively with treatment and criminal justice systems. Proficiency in computer applications or other automated systems such as spreadsheets, Microsoft Office, calendar, email, and/or database software necessary to perform work assignments.
