Practice Setting: Office/Clinic & Hospital & Practice Call (GYN only)
Reason for Coverage: Maternity Leave
Dates of Coverage: 5/1/25-11/15/25
Call Ratio: 1:4 (ED); 1:2 (Practice)
Average Number of Scheduled Patients Per Day:30+
Number of Physicians in Practice: 1 Physician; 1 APP
Will Other OB/GYNs Provide Coverage During Locums Coverage Dates (List Names): Yes - Dr. Angelle Brebnor
How Many APPs Provide Coverage: 1 with physician backup
Number of Exam Rooms: 8
Number of Exam Rooms: 8
Onsite Lab? Yes
Onsite Ultrasound? Yes
Describe Office Equipment: LEEP Machine
Describe Support Staff: 1 MA per provider, 2 LPN's to manage surgical scheduling & patient follow up calls
What Percentage of Practice is Low Risk Pregnancy: No longer seeing new OB patients 10% remaining
What Percentage of Practice is GYN: 95%
Documentation System/EMR: EPIC
Facility will reimburse for the following costs: Mileage Billed at the IRS Standard Rate*** Provider Must be Local - Only Mileage is Reimbursed

Required Obstetrics Procedures:
Low Risk Pregnancy (i.e. History of Twins, Toxemia, Pyelonephritis, etc.)
FHR Interpretation - NST/CST/FSE","OB ultrasounds - Performance of Interpretation
OB ultrasounds - Performance of Level I/Screening (Viability)
OB ultrasounds - Performance of Level II/Targeted (Anatomic)
Gynecology Required Procedures:
Office/Minor Gynecology -IUD, Cystoscopy, Colposcopy, Cryosurgery, LEEP, etc.
Surgical Gynecology - Open
Operative/Major (Includes Diagnostic) - Ectopic Pregnancy, Ovary Removal, Cystectomy, Extensive LOA's
