Nurse Practitioner

Overview Signify Health, part of CVS Health®, is hiring a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to perform in‑home health evaluations. Clinicians will visit patients in their homes, conduct physical assessments, review medical history and medication, and order diagnostic tests when indicated. Responsibilities Conduct in‑home visits lasting approximately 45 minutes, engaging patients one‑on‑one. Perform medication and medical history reviews, physical examinations, and order additional diagnostic tests (e.g., diabetic eye exams, spirometry) as authorized by the health plan. Use an iPad and clinical workflow tools to complete evaluations. Provide patient education and support to improve health outcomes. Be flexible to travel locally and within licensed states and conduct virtual visits when needed. Required Qualifications Active, unrestricted license(s) in the coverage area(s) (Nurse Practitioner or Physician Assistant). Board certification required. Multi‑state licenses may be required. Preferred Qualifications Experience performing in‑home medical assessments. Ability to cover primary locations in Penobscot, ME. Additional certifications relevant to diagnostic and preventive services. Work Environment Clinicians work primarily in private residences. Environments may include household pets, secondhand smoke, or other allergens; candidates must be able to perform duties safely in such settings. Compensation Hourly pay range: $37.66 - $81.11.
