Choice Health At Home is seeking a Home Health RN Case Manager in Sierra Vista, AZ. This full-time role requires assessing and planning patient-centered care delivered in patients’ homes, coordinating with interdisciplinary teams, and supporting recovery or disease management.

You will perform comprehensive assessments, plan and document care, educate patients and families, and coordinate services to ensure continuity of care and high-quality outcomes.
