Physical Therapist

Provide skilled physical therapy services to patients in a home health setting, ensuring optimal recovery and functional independence. The therapist will work independently within a designated territory while complying with agency policies and healthcare standards.

Responsibilities:

• Assess patients' physical conditions and develop individualized treatment plans.
• Implement therapy interventions to promote mobility, strength, and pain management.
• Document patient progress and clinical notes accurately using Oasis and other required systems.
• Communicate effectively with patients, family members, and interdisciplinary team members.
• Maintain compliance with travel and documentation regulations, including timely updates.
• Adhere to all safety protocols and licensing requirements, including active CA license and BLS certification.

Qualifications:

• Hold an active California Physical Therapy license or be in the process of obtaining it.
• Possess a minimum of 1 year of home health therapy experience.
• Demonstrate proficiency with Oasis documentation and home health workflows.
• Be licensed in California and have a valid driver's license with reliable personal transportation.
• Have experience in home health care practice and patient assessment.
• BLS (AHA) certification is required.
