Registered Nurse - Hospice

Job Description

Job Summary: The RN - Case Manager provides comprehensive nursing care and case management services to patients in their homes, ensuring coordinated, quality, and cost-effective care. The role involves admitting patients onto services, assessing their needs, and overseeing clinical and support staff to optimize patient outcomes.

Responsibilities:

• Assess patients' physical and psychosocial conditions through systematic evaluations and develop individualized care plans.
• Coordinate, monitor, and adjust care delivery in collaboration with patients, families, and interdisciplinary teams.
• Oversee care provided by Home Health Aides and CNAs, ensuring adherence to established plans of care.
• Communicate effectively with healthcare providers, community agencies, and support services to ensure continuity of care.
• Educate patients and families on health management, symptom control, and appropriate interventions.
• Maintain accurate, complete patient records in compliance with agency standards, reimbursement, and OASIS requirements.

Qualifications:

• Hold a current RN license in the State of Montana, with BSN preferred.
• Possess a valid driver's license, auto insurance, and reliable transportation.
• Have at least 2 years of recent acute care, home care, or hospice clinical experience.
• Demonstrate clinical skills such as venipuncture, wound care, catheterization, or parenteral therapy.
• Experience with HCHB is strongly preferred.
