Stony Brook CPMP is seeking a Community Health Navigator in Commack, NY to support administrative operations and patient-facing interactions within an interdisciplinary Care Management team.
The role requires coordination with Care Managers to ensure adherence to care plans, assist with referrals, and help patients access community resources and transportation. Monday–Friday, full-time, with a salary range of $25.27–$31.58 per hour.
