Location: Petaluma/Santa Rosa, CA 95401
Setting: Home Health
Assignment: 13 weeks | Start: ASAP
Schedule: 40 hrs guaranteed
Weekly Pay: $2,583–$2,870/week

Job Description

We are seeking a Travel PT to join a collaborative Home Health rehabilitation team. You will provide high-quality patient care, develop individualized treatment plans, collaborate with the interdisciplinary team, and maintain accurate documentation while helping patients achieve their rehabilitation goals.

Responsibilities

• Evaluate and treat patients
• Develop individualized treatment plans
• Document patient progress
• Collaborate with physicians, nursing staff, and rehab team
• Educate patients and caregivers

Qualifications

• Active PT license
• Graduate of an accredited program
• Minimum 1 year of experience preferred
• HH experience preferred

Requirements

• Current BLS (if required)
• Strong communication skills
• Ability to work independently and as part of a team
