A clinical pharmacist with expertise in infusion therapy is sought for an 8-12 week contract position in Spring, TX, starting as soon as possible. This role focuses on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications primarily in a home infusion setting. The pharmacist will serve as a key member of a multidisciplinary team, ensuring effective therapy management, patient education, and regulatory compliance.

Schedule:

• Monday to Friday, 8:30 AM – 5:00 PM
• May include participation in on-call rotation

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions under sterile conditions in accordance with state and federal regulations.
• Monitor patient lab values, calculate pharmacokinetic dosing (e.g., vancomycin), and adjust Total Parenteral Nutrition (TPN) regimens.
• Provide detailed education and counseling to patients and caregivers on infusion pumps, sterile techniques, and catheter care.
• Coordinate care with physicians and home health nurses to optimize treatment outcomes and reduce hospital readmissions.
• Collaborate with intake, referral, reimbursement, nursing, and pharmacy teams to support smooth patient transitions.
• Ensure compliance with all applicable regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Manage pharmacy inventory, including refrigerated products, and oversee packing and shipping of medications to patient homes.

Desired Experience and Qualifications:

• Current and valid Texas pharmacist license in good standing
• 1-3 years of experience in infusion specialty pharmacy or hospital pharmacy settings, including primary work in the pharmacy IV room
• Familiarity with IVIg therapy is a plus
• Understanding of specialty therapy payer requirements, reimbursement procedures, and contract obligations
• Proficiency with CareTend system preferred
