## Social WorkerApplylocations: The Pavilion at Williamsburg Placetime type: Full timeposted on: Posted Todayjob requisition id: JR125565Social Worker | The Pavilion at Williamsburg Place | Williamsburg, Virginia**About the Job:**The Social Worker I plans, directs, defines and coordinates individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance abuse and physical and emotional abuse. Conducts individual and group therapy, crisis intervention, case management, and education.**Roles and Responsibilities:**· Conducts social history assessment of the patient through observation and patient/family interview, obtaining information on the patient’s needs, skills, interests and limitations for the development and implementation of the treatment plan.· Coordinates and plans programs and activities to meet patient’s social and emotional needs.· Promotes recovery, symptom reduction and the achievement of the highest level of social functioning.· Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle.· Participates in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan and ongoing discharge plan.· Communicates or aids family members in understanding and supporting the patient.· Manages a caseload and maintain accurate and timely documentation to assure patient records comply with facility and accreditation standards.· Facilitates education, process groups, support groups and referrals as requested.· Assists in the development of effective treatment plans that apply therapeutic principles appropriately while ensuring safety at all times for the patient.· Bachelor's degree in Social Work from an accredited college or university required.· Actively pursuing a master’s degree in social work.· One or more years’ experience working with patient population preferred.· Eligible for state licensure as one of the following: Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Certified Addictions Counselor (CAC), Registered Addiction Counselor (RAC), as required by the state in which the facility is operating.Why The Pavilion at Williamsburg Place?The Pavilion at Williamsburg Place offers a comprehensive benefit plan and a competitive salary commensurate with experience and qualifications.
