Home Health Registered Nurse (RN) – Mississippi

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Weekends: Some weekends may be required
Hours: 40 hours not guaranteed
On-Call: None
Location: Mississippi
Mileage: $0.49/mile
Productivity: 30 points/week

Job Summary

We are seeking a Registered Nurse (RN) to provide high-quality home health care and case management. The RN will assess patients, develop and implement care plans, coordinate with healthcare providers, and monitor ongoing patient needs.

Responsibilities

• Assess, plan, implement, and evaluate patient care.
• Provide health maintenance, disease prevention, and case management.
• Coordinate care with physicians and other healthcare professionals.
• Monitor medications, physician orders, and changes in patient condition.
• Maintain accurate nursing documentation and required reports.
• Schedule and coordinate health-related appointments and follow-up care.
• Supervise LPN/LVN staff as needed.
• Support hospital discharge coordination and continuity of care.
• Train staff on health, safety, and patient-specific care requirements.
• Ensure proper infection control, equipment, and supply standards.

Qualifications

• Active Mississippi RN license required; compact licenses accepted
• Home health experience required
• HCHB experience required
• OASIS experience required
• Strong communication, organization, and clinical assessment skills
