Job Summary As a Health Navigator Team Lead, you will manage Health Navigation staff, handle crises, provide support to clients with complex behavioral health issues, ensure client safety, and connect clients to high‑quality evidence‑based providers and Veterans Administration Medical Center services. The role focuses on coordinating services for veteran households, preventing homelessness, and facilitating rapid re‑housing and transitional supportive services. Responsibilities Coordinate collaborative efforts with the SSVF Program and develop a schedule to visit VA, shelters, and other sites where homeless and low‑income veteran families congregate. Independently manage a caseload of complex, multi‑need clients. Provide leadership for the Health Navigator program and act as a backup resource for case management teams. Develop inter‑agency relationships to enhance referral processes and maintain good landlord/tenant relations. Review client files, provide guidance, resolve issues raised by Health Navigators, and conduct weekly team meetings. Monitor the availability and effectiveness of services provided to each veteran household; determine quality and improve service delivery. Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan. Coordinate and maintain case documentation, prepare reports, and submit documentation required by the SSVF grant. Facilitate communication with local, county, and regional agencies and attend public meetings to present the Health Navigation program. Analyze and evaluate program data, prepare reports, and recommend improvements to the Health Navigator program. Screen potential participants for program eligibility using approved forms and verify participant information and income status. Provide service coordination and arrange support for veterans transitioning to or maintaining permanent housing. Travel to housing sites to monitor progress, address health, behavioral, and life‑skills issues, and communicate with clients and service teams. Work with clients, families, and housing agencies to revise service plans and develop placement, stabilization, and follow‑up plans. Ensure participant reassessments are completed per SSVF requirements and that financial assistance is documented appropriately. Participate in program and professional supervision, attend required meetings and trainings, and contribute to team‑based collaborative planning. Complete all required reporting in AWARDS and other systems. Qualifications Clinical license preferred but not required. M‑Aster’s level social worker or equivalent education with experience in counseling psychology, nursing, rehabilitative counseling, or related human services field. Experience working with homeless individuals and families, including veterans, is a plus. Comfortable in environments with constantly evolving responsibilities. Passion for connecting individuals with excellent care and ability to provide short‑term support in person, over the phone, and via email. Other Requirements Must drive personal vehicle for company business; valid driver’s license and vehicle liability coverage required; vehicle use may include van. Must pass an acceptable criminal background check, OIG exclusion check, negative drug screening, and hold CPR/First Aid certification. Physical Requirements Ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies. Physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear, and perform basic and light home maintenance activities, and operate office equipment. Operating office equipment requiring continuous or repetitive hand/arm movements.
