Registered Nurse – Home Health

Schedule: 8 hours per shift

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience: Required
• OASIS Experience: Required
• CHPN certification: Preferred
• Proof of current CPR certification: Required
• Current driver's license: Required
• Must have reliable transportation and insurance: Required
• 1-2 years experience as a Registered Nurse in a clinical care setting: Required
• 2+ years home health experience: Required

Roles & Responsibilities:

• Assess home health and hospice patients to identify their physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
• Educate patients and their families regarding care, diet, medications, and home safety, using appropriate teaching methods.
• Supervise and provide clinical direction to junior staff to ensure quality service delivery.
• Monitor assigned cases for compliance with third-party payer requirements.
• Serve as a liaison between patients, families, and other healthcare professionals to facilitate communication and coordination of care.

Compliance / Notes:

• Performs on-call responsibilities and provides on-call services to patients and families as assigned.
• Attends interdisciplinary team meetings as necessary.
