Contract | Days | Monday – Friday, 8:00 AM – 5:00 PM | On-Call Required | 13-Week Assignment

Key Responsibilities

• Provide physical therapy services to patients in a home health setting
• Conduct evaluations and develop individualized treatment plans
• Meet productivity expectations of 30 points per week
• Participate in on-call rotation as required
• Complete documentation accurately using HCHB and OASIS platforms

Required Qualifications

• Active Physical Therapist (PT) license in the state of Ohio
• Minimum of 12 months of home health experience within the past 3 years
• Proficiency with HCHB documentation system
• OASIS competency required
• Ability to meet a 40-hour per week commitment

Preferred Experience / Skills

• Experience working in a home health environment beyond the minimum requirement
• Strong time management and organizational skills in an independent work setting
