Admissions Coordinator

Job Summary

The Admissions Coordinator will be responsible for managing the admission process, as well as handling transfers, discharges, and fall reporting for residents. This role requires a thorough understanding of admissions protocols and strong organizational skills to ensure smooth operations.

Responsibilities

Oversee the admissions process and ensure compliance with relevant regulations and standards.
Manage transfers and discharges, ensuring proper documentation and communication with relevant staff.
Conduct fall reporting in accordance with established procedures and protocols.
Maintain accurate and organized records related to admissions, transfers, and discharges.
Collaborate with nursing staff and other care team members to coordinate care for residents.
Participate in training and development activities to enhance knowledge and skills in admissions processes.

Staffing Information

The facility will maintain a caregiver-to-resident ratio of:

1:35 at night, with a medication pass at the beginning of the shift and assistance from up to 2 Certified Nursing Assistants (CNAs).
1:35 during the day, with support from a medication aide, a nurse responsible for treatments, up to 3 CNAs, and potentially 1 Bath Aide (Monday through Friday).

Work Schedule

The work week runs from Sunday through Saturday, with guaranteed hours specified based on the shift structure.
