Care Manager RN - ED
As an Inpatient Registered Nurse (RN) Care Manager in the Emergency Department (ED), you will deliver patient?centric care management services for at?risk patients in an acute hospital setting. Major responsibilities include:
- Assessment and care planning for patients with complex needs
- Coordination of care across departments and transitions
- Resource utilization review and management
- Discharge planning and follow?up to ensure continuity of care
- Documentation of interventions and ensuring regulatory compliance
- Advocating on behalf of patients and families
You'll work alongside care?management assistants and a leadership team in a unionized setting that values accountability, shared success, and excellence in patient care.
Qualifications Required:
- Associate's Degree in Nursing (ADN) or Nursing diploma upon hire
- Washington Registered Nurse (RN) license upon hire
- Minimum 2 years of acute?care experience in a Medical?Surgical or inpatient setting
Skills & Experience Required
- Strong clinical assessment and care?planning skills
- Proven ability in coordinating multidisciplinary teams and facilitating transitions of care
- Solid documentation practices and understanding of regulatory requirements (e.g., Joint Commission standards)
