This role includes initial evaluations , Plan of Care development , and ongoing treatment visits , with compensation structured per visit or hourly depending on the case and service provided . This position is ideal for therapists who are confident working independently, experienced in home health, and skilled in completing thorough clinical documentation that supports patient progress and care planning.
