Rehabilitation Therapy - Physical Therapist

PT Location: Clackamas, OR Schedule: 8:00AM - 5:00PM x 5 Term: 13-week assignment with the potential for extension Pay Rate: $54 - $58/hr

Astrya Global, a San Diego based medical staffing agency, is hiring a PT for a temporary assignment in Clackamas, OR. These positions are 13-week contracts with the ability to extend.

Responsibilities for a PT:

Complete required documentation directly with ATI outside of standard clearance packet before being cleared to start assignment Obtain permanent license as soon as possible if starting with a compact license Must have permanent license if working in SC; compact licenses are not accepted for SC positions Maintain active license as required for the position Work 32 guaranteed weekly hours during holiday weeks Work scheduled day shift hours Adhere to a 30-day notice requirement Follow dress code requirements as provided during first day orientation Utilize facility-provided equipment as outlined during interview Meet experience requirements specific to the position setting Comply with all credentialing requirements including contract QA Self-report any separation history from previous employment as required for eligibility review Adhere to all facility policies and procedures for patient care delivery
