Care Manager RN Per Diem Day

Description Care Manager RN at Providence Queen of the Valley Medical Center in Napa, CA. This position is Per Diem and will work 8‑hour day shifts. Responsibilities The RN Case Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. The RN Case Manager assists patients in the utilization of appropriate health care services. Position may require utilization management and discharge planning, and chronic disease case management based on inpatient settings. Required Qualifications Recent completion of a post‑graduate level curriculum focusing on case management, or equivalent. Current clinical experience of 2 years in an acute care hospital setting, or 1 year current experience in an inpatient or outpatient case management setting. California Registered Nurse license upon hire. 2 years case‑management, discharge planning, or utilization management experience in the acute, sub‑acute, home‑health setting or managed‑care environment. Preferred Qualifications Bachelor’s Degree in Nursing, required to be obtained within 3 years of hire date. 2 years case‑management, discharge planning, or utilization management experience in the acute, sub‑acute, home‑health setting or managed‑care environment. 3 years acute care experience.
