Requirements:

• 2 years of experience as a RN

• Qualified applicants MUST have at least 2 years years of experience in Home Health

• Qualified applicants MUST have OASIS and Homecare Homebase EMR system experience

• Valid RN license

• Be willing to obtain Florida licensure

Responsibilities: As a healthcare professional in a Clinical setting, you will be responsible for delivering high-quality home health nursing care while supporting ongoing plan-of-care needs.

• Assess and manage patient care needs in Home Health, including completing OASIS documentation and coordinating visits based on clinical priorities.

• Use Homecare Homebase EMR to accurately chart nursing notes, care plans, and outcomes in alignment with home health standards.

• Apply strong RN skills to support care coordination, patient education, and compliance with quality measures for Home Health services.
