Clinical Account Liaison for SNF Partners
Senior Care Therapy
Senior Care Therapy is seeking a Clinical Account Liaison to serve as the embedded clinical liaison for a portfolio of approximately 20 SNF partners in the Northeast. You will identify residents who could benefit from psychiatric and psychological services and connect them to SCT’s clinical team in a timely, medically necessary manner. Working with DONs, social services, MDS coordinators, pharmacists, and unit managers, you’ll educate facility staff about SCT services, support appropriate #J-18808-Ljbffr Senior Care Therapy
