Registered Nurse - Case Manager

Registered Nurse (RN) Responsibilities: Plan, organize, and direct home care services. Build from community resources to meet the needs of individuals and families within homes and communities. Collaborate with team members to achieve positive patient care outcomes. Requirements: Registered Nurse with experience in nursing, with emphasis on community health education/experience. Home Health experience preferred, but not required. Job Type: Full-time COVID-19 considerations: Our organization has plenty of PPE to protect our employees, patients and families. Licensed Practical Nurse (LPN) Responsibilities: Provide direct patient care under the supervision of a registered nurse. Follow the plan of care and provide treatments. Work collaboratively with the team to meet positive patient care outcomes. Requirements: Licensed Practical Nurse experience in home health preferred. Per-Need Registered Nurse (PRN RN) Responsibilities: Be available for patient care a minimum of four days per month. Requirements: Registered Nurse with ability to plan, organize, and direct home care services under community health emphasis. Home Health experience preferred, but not required.
