Job Description

Effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care, developing appropriate care plans in accordance with all regulatory and clinical standards. Utilize standardized assessments and evidence-based practices to support clinical interventions. Administer various procedures, including manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, and wound care. Assess patient progress and adjust treatments as needed, documenting all clinical and regulatory information accurately in electronic records. Evaluate treatment outcomes and modify services accordingly, making appropriate referrals based on patient evaluations. Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Communicate effectively with the healthcare team regarding patient progress, barriers, and treatment plans. Prepare comprehensive discharge summaries in compliance with regulations. Participate in required meetings such as patient care conferences, utilization reviews, and rehabilitation meetings. Support supervision of physical therapist assistants, rehabilitation aides, or students when applicable. Adhere to the American Physical Therapy Association (APTA) standards, state practice acts, ethics, and conduct during professional practice.
