Our work is rooted in compassion, dignity, and a shared commitment to helping others thrive. Lutheran Services Carolinas is looking for an IAFT Care Coordinator. The IAFT Care Coordinator works intensively with the IAFT team by providing ongoing support that serves to stabilize the placement and facilitate behavioral change necessary for successful transitions for severely emotionally disturbed youth in care. This includes facilitating the setting of placement goals and desired outcomes established by the placement team. The IAFT Care Coordinator is responsible for writing and monitoring behavioral intervention treatment plans, monitoring progress according to the level of care, and documentation and planning transitions for discharges. Must be able to commute to Winston Salem, NC, occasionally. Essential Functions Follow all policies and procedures established by Lutheran Services Carolinas. Identify, link, and coordinate initial and ongoing systems of care and wrap services based on assessment activities. Assist in facilitating placement matches; consult with team members and assist with matching and obtaining placement paperwork, including clinical information and contractual documentation. Attend daily and weekly clinical team meetings, regularly updating the team on progress to deliver the clinical interventions. Deliver and perform clinical interventions as directed to child and family by the clinical team. Write and monitor behavioral intervention treatment plans, monitor progress according to the level of care, and plan transitions for discharges. Maintain documentation in the youth’s file and IAFT parent file per state and federal standards; complete documentation for all services for both billable and non‑billable services. Utilize the database to input required data as needed. Complete all placement and attendance forms required for billing and ensure all authorizations remain current. Provide ongoing support and supervision to IAFT parents who carry out therapeutic interventions for the youth; assist with crisis interventions by diffusing the current crisis, evaluating its nature, and intervening to reduce reoccurrence. Assist in marketing LSC programs with potential referral sources and respond independently and promptly to marketing and community relations needs. Manage regional team operations under the guidance of the Regional Team Manager, including facilitating placement goals, recruitment and licensing, budgeting, and promoting LSC within the child welfare community. Serve on committees and projects at LSC. Utilize Person Thinking Planning Model and assist with the creation of Person‑Centered plans. Respond to other duties as requested. Qualifications Bachelor’s degree in a field related to social work and two years of related experience, two of which must be in child welfare; OR a Bachelor’s degree in a non‑related field with five years of related experience, at least two in child welfare; OR a Master’s degree in a human services related field and one year of experience in child welfare. Must have specific knowledge of child welfare systems. Skills in group and team facilitation, completing assessments, and writing behavioral treatment plans. Self‑motivated, good time management skills, and ability to work independently. Flexible and able to work irregular hours. Specialized Knowledge, Licenses, etc At least 21 years of age, possess a valid driver’s license and reliable transportation. Knowledge of family systems, childhood developmental and attachment theory, psychopathology, behavior management, and psychotropic medications. #J-18808-Ljbffr
