We partner closely with our patients to provide comprehensive, patient-centered care that supports long-term health and wellness.About The JobResponsible for managing care for complex gastroenterology patients to promote effective education, self-management support, care coordination, and timely healthcare delivery. Collaborates with patients, providers, and interdisciplinary teams to develop individualized care plans and improve patient outcomes.What You’ll DoProvide care coordination and case management for patients with complex healthcare needs.Collaborate with providers and interdisciplinary care teams to develop individualized care plans.Educate patients on disease management, medications, nutrition, and self-management strategies.Coordinate transitions of care, including hospital discharges, specialty referrals, and follow-up care.Utilize population health tools and electronic medical records to identify and manage patients requiring additional support.Promote patient safety, quality outcomes, continuity of care, and preventive health initiatives.Who You AreAssociate Degree in Nursing (ASN) or equivalent from an accredited institution required.
