Responsibilities

• Deliver direct patient care under supervision with accuracy and empathy.

• Document patient observations and care provided in electronic records daily.

• Monitor and report changes in patient condition to team lead promptly.

• Assist with activities of daily living while maintaining patient safety.

• Adhere to policies, procedures, and regulatory requirements consistently.

• Participate in ongoing learning and skill development aligned with growth goals.

• Collaborate with the healthcare team to achieve measurable performance targets.

• Demonstrate accountability for personal outcomes and contribute to team success.

• Support care planning by collecting routine data and feedback.

• Maintain cleanliness and safety of the patient care environment.

• Utilize care pipelines and metrics to identify opportunities for territory or area improvement.

• Assist with patient education and discharge planning when appropriate.

Experience/Qualifications

• Active CNA license with current certification requirements fulfilled.

• Accurate documentation of observations in electronic health records daily.

• Adhere to policies, procedures, and regulatory requirements at all times.
