What You'll Do You'll manage a panel of medically complex Medicare patients by delivering comprehensive, patient-centered care across clinic, dialysis center, and home settings. Responsibilities include: Provide longitudinal primary care for patients with CKD and ESKD Perform comprehensive health assessments and annual health risk assessments Develop individualized care plans that support improved health outcomes Educate patients and families about diagnoses, medications, and disease management Coordinate care with nephrologists, primary care providers, specialists, caregivers, and community resources Manage chronic conditions, medication regimens, and post-discharge follow-up Help prevent unnecessary emergency department visits and hospital readmissions Accurately document clinical encounters and HCC coding Order and interpret diagnostic tests as appropriate Practice independently while collaborating closely with physicians and an integrated care team Maintain compliance with HIPAA and all regulatory requirements This position offers a blend of: Dialysis center visits Nephrology clinic visits Occasional telehealth appointments Travel is primarily within your assigned territory, with occasional coverage in nearby regions based on business needs.
