MSW: Dialysis Care & Family Support Specialist
Fresenius Medical Care
Fresenius Medical Care is seeking an entry-level Master Social Worker (MSW) to provide psychosocial services to patients in our Waldorf facility. You will work with the healthcare team to support adjustment, rehabilitation and quality of life for patients with CKD, including education and family support. You will assess psychosocial needs, develop care plans with the team, and refer to community resources as appropriate, while upholding policy and quality initiatives. #J-18808-Ljbffr Fresenius Medical Care
