Admission Nurse

Job Summary Admission Nurse responsible for initiating and resuming high‑quality home health services for patients referred to the agency in Queens, NY and occasionally Brooklyn. Key Responsibilities Create the initial comprehensive and interdisciplinary patient centered home health plan of care; update the plan in collaboration with physician or authorized practitioner (AP). Ensure agency compliance with regulatory and organizational requirements. Demonstrate expert knowledge unique to the practice of community health nursing. Provide high quality clinical care and teaching. Qualifications Diploma in Nursing required; Baccalaureate Degree highly preferred. Current RN License in NY in good standing. Minimum one year of clinical experience in a Certified Home Health Agency. Knowledge of Microsoft Office and experience documenting in a clinical software system preferred. Driver’s License required in all boroughs except Manhattan. Strong assessment skills, clinical judgment, and critical thinking abilities. Ability to work independently in a community setting.
