Nurse Practitioner (NP) / Physician Assistant (PA) – Home-Based Primary Care

Location: Bronx/Westchester or Brooklyn/Staten Island, NY

Compensation: $145,000 - $220,000

Employment type: Full-time

Job Overview:

Provide high-quality, home-based primary care to high-risk and medically complex patients in a value-based care program. Collaborate with an interdisciplinary team to develop personalized care plans, improve patient outcomes, and reduce avoidable hospitalizations. Make a meaningful impact by delivering compassionate, coordinated care in patients' homes.

Responsibilities:

• Provide home-based primary care for high-risk, medically complex patients.
• Diagnose and manage acute & chronic conditions.
• Perform assessments, prescribe medications, and order/interpret labs.
• Develop personalized care plans and educate patients/caregivers.
• Collaborate with interdisciplinary care teams.
• Maintain accurate EMR documentation.
• Participate in rotating after-hours call.

Qualifications:

• NP or PA from an accredited program.
• Active New York NP/PA license , National Certification & DEA.
• 3+ years of clinical experience.
• Geriatrics, home-based primary care, or chronic disease management preferred.
• EMR/EHR proficiency.
• Spanish bilingual preferred.
