Facility: Little Chute Health Services
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a licensed Physical Therapist (PT) for a PRN opportunity at Little Chute Health Services in Little Chute, Wisconsin.

Join a collaborative rehabilitation team providing high-quality physical therapy services to adult and geriatric residents in a skilled nursing facility. This PRN position offers flexible scheduling while allowing you to make a meaningful impact on patients’ mobility, independence, and quality of life.

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services to adult and geriatric patients.
• Improve mobility, strength, balance, endurance, and functional independence.
• Collaborate with physicians, nursing staff, and the interdisciplinary rehabilitation team.
• Complete accurate and timely clinical documentation.
• Educate patients and caregivers on mobility, safety, and home exercise programs.
• Deliver compassionate, patient-centered care.

• Graduate of an accredited Physical Therapy program.
• Current Wisconsin Physical Therapist (PT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
