Join a respected healthcare team as a Licensed Bachelor Social Worker (LBSW)! You'll support patients and families by assessing needs, coordinating care plans, assisting with discharge planning, connecting individuals with community resources, and collaborating with healthcare professionals to ensure safe, effective, and patient-centered care throughout the hospital stay.

Shifts

• Day Shift
• 8-hour shifts

Responsibilities

• Coordinate patient care and discharge planning.
• Assess patients' psychosocial and support needs.
• Connect patients and families with appropriate community resources.
• Collaborate with physicians, nurses, case managers, and other healthcare professionals.
• Help ensure patients receive the appropriate level of care.
• Advocate for patients to promote safe and effective care transitions.
• Maintain accurate documentation in the medical record.
• Educate patients and families about available healthcare and community services.
• Support quality, safety, and positive patient outcomes.

Requirements

• Active Kansas LBSW (Licensed Bachelor Social Worker) license
• Bachelor's Degree in Social Work or a related field
• Hospital social work experience preferred
• Strong communication and interpersonal skills
• Ability to assess patient needs and coordinate care
• Experience working with patients, families, and interdisciplinary healthcare teams
• Excellent organizational and documentation skills

Salary- Starting at $24.77+/hour (based on education and experience)
