Hospice Registered Nurse- Full Time, Days, Home Care, Morris County NJ

Job Overview Location: Basking Ridge, New Jersey (on-site). Job type: Full-Time. Hours: 8:00 AM - 4:00 PM, weekends and on‑call as required. Salary range: $40.00 – $70.40 per hour, with entry level starting at $43.00/hr and mileage reimbursement ($0.725 per mile). Internal referral bonus: $7,500. Responsibilities Perform patient assessments and accurately determine appropriateness for hospice care, learning needs, and progress toward goals. Develop, implement, and evaluate individualized plans of care for patients and families. Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement. Complete discharge assessments: perform laptop transfer of data twice daily, complete admission assessment and transfer within 24 hours, and complete re‑visit documentation and transfers. Use age‑appropriate teaching methods for patients and families: evaluate teaching plans, revise as necessary, and document patient education tools used. Assume responsibility for case management: revise care plan, prioritize care needs, and communicate progress toward goals with the interdisciplinary team. Ensure compliance with hospice regulations and insurance reimbursement guidelines. Perform HHA supervisions per standards and follow scheduling processes. Maintain a safe environment: follow infection control measures, demonstrate pharmacology knowledge, reconcile medications, and maintain current medication orders. Participate in monthly staff meetings, PI meetings, and other committee activities as requested. Assist with performance improvement: review records, gather data, attend UR meetings, mentor new employees, and cross‑train to learn new skills. Support agency growth: provide orientation to new employees and cross‑train, accepting other assignments as required. Qualifications Current New Jersey RN license. Valid, unrestricted driver’s license with a clean driving record. Associate’s Degree in Nursing (ASN). Basic Life Support (BLS) certification. Preferred: Bachelor’s degree in Nursing; if holding an ASN, must complete BSN studies within five years of start date. Preferred: Master’s degree in nursing or related field. Preferred: At least 1 year of nursing or relevant experience. Preferred: ACLS certification.
