Role Description This is a full-time on-site role as an Admission Nurse at Life Hospice, located in Livingston, NJ. Evaluating and admitting new patients to hospice care Conducting initial patient assessments Creating personalized care plans Coordinating with other healthcare professionals to ensure consistent and effective care Providing palliative care Educating patients and families about the hospice process Ensuring compliance with all relevant regulations and standards Qualifications Nursing and Medicine skills Experience in Palliative Care Home Care and Long-term Care skills Excellent communication and interpersonal skills Strong organizational and time management abilities Ability to work independently and as part of a team Current nursing license in the state of New Jersey Prior hospice or home health experience is a plus #J-18808-Ljbffr Life Hospice
