Patient Onboarding Specialist

Your Role Here’s what you will be doing: Orchestrate patient scheduling across all service lines, balancing urgency, clinical priorities, and patient needs in a dynamic environment. Manage the referral process end-to-end by interpreting and processing partner referrals, scheduling patients appropriately, and documenting outcomes in partner EHRs and internal systems. Serve as the first problem-solver for incoming calls from patients, partners, or other stakeholders, resolving issues directly or connecting to the right team member. Handle complex pre-visit steps including insurance verification, consents, records retrieval, questionnaires, and identifying barriers such as transportation to support the care team. Deliver a supportive experience combining empathy, resourcefulness, and technology to assist patients and instill confidence in partners. Ensure timely communication by returning calls promptly, documenting outcomes accurately, and keeping patients and care teams informed. Collaborate with team members to refine workflows, design playbooks, and improve systems and tools as the department scales. Maintain HIPAA and data-privacy standards while ensuring meticulous and timely documentation. Consistently meet service benchmarks related to responsiveness, conversation rates, visit readiness, and documentation quality, while seeking improvements.
