Resident Care CoordinatorThe Enriched Living Center is a 52 bed long-term residence for people with a diagnosis of mental illness. The Resident Care Coordinator will assist the Enriched Living Center program by providing leadership and mentoring to the Resident Counselor I/ II staff. Assist with the overall rehabilitation and the improved quality of life of the residents.Essential Job FunctionsServe as assigned residents' primary counselor for service provision.Maintain current individual resident records for any assigned residents. This includes meeting with all Primary Counselors to develop and review Service Plans(every 3 months) and Monthly Summaries (30 days)for Residents ensuring they are measurable and completed on time.Document progress notes for any residents you have communication or coordinate care with.Conduct Resident assessments and Medication assessmentsCheck Daily Medication Audits, if not completed communicate with Staff to ensure they are done and they are completed within 24 hours.Confirm that Resident Room checks are done on Thursdays and Room cleanings are completed by Mondays.Provide assistance in ADL skills.Employ as needed (with residents): informal counseling, mediation, conflict resolution, crisis counseling, etc.Serve as a member of the ELC On-Call administrative team.Assist Residents with Vocational Rehabilitation/Employment refer to the Rescue Mission Community Resource Room as needed.Coordinate with direct care staff to implement and maintain monthly activities calendar for residentsResponsible for Maintenance of resident documentationDrive residents in a department vehicle, as needed.Assist with the overall rehabilitation that will improve the quality of life for residents.Minimum Job QualificationsHigh School diploma with 5 years of experience with a mental health population and additional pertinent certificates/training in related medical or psychiatric fieldORAssociate Degree in Human Service or related field with 3 years of experience with a mental health population.Minimum of two (2) years management and supervisory experience preferredAbility to provide direction/model, delegate, train and supervise others.Intermediate skills with Microsoft Word, Excel and Outlook.Clean/valid NYS Driver's License RequiredEnthusiastic support of our Mission Statement.Core CompetenciesDemonstrates wisdom, good decision making, analytical and problem solving skillsMaintains good attendance and punctualityCommitment to maintaining professional conduct and appearance and upholds the integrity of the Rescue Mission's standardsAbility to set appropriate boundaries with residentsMeets deadlinesAbility to maintain confidentiality and HIPAA privacy laws.Team playerStrong and proven management and leadership skillsPromotes harmonious working relationshipsDemonstrates effective verbal, written and listening communication skills and is not easily misunderstoodAbility to remain calm, patient and professional in a fast paced environmentAbility to resolve conflictAbility to work with little or no supervisionMust be a motivated self-starterAbility to multi-task, plan & prioritize tasks and respond to situations effectivelyPhysical Job RequirementsThe tasks described signify many of the physical requirements of this position, based on a Time Weighted Average of an 8-hour workday.Lift/move up to 25 lbsTwisting at the torso in stationary positionBending at waist using legs to lift heavy objects up to 35 lbsBending at the waist to perform work at that levelStanding/sitting for extended periodWalking up and down stairsTyping on a computer and using a mouseWalking through facility and campusOther Work PerformedDriving residents in the Rescue Mission Vehicle e.g. department van.Every employee of the Rescue Mission is required to complete annual mandated trainings.Status/Hours:Full Time, 40 hours per week Mon-Fri 7am-3pm plus On-Call ResponsibilitiesPay Rate:$21.00-23.00 per hour, depending on experienceBenefits:We offer a comprehensive benefit package for eligible employees to include medical, dental, life insurance, 401(k), voluntary benefits, paid time off, holidays and paid training.The Rescue Mission of Utica is an equal opportunity employer encouraging individuals with disabilities and veterans to apply.
