Human Touch Home Health is seeking an experienced Registered Nurse (RN) to provide one-on-one home health services throughout Washington, DC .

What You'll Do

Perform comprehensive nursing assessments and develop individualized care plans
Provide skilled nursing care, medication management, wound care, and patient education
Monitor patient progress and communicate changes to physicians and the interdisciplinary team
Coordinate care with therapists, social workers, and caregivers
Complete documentation accurately and on time

What You'll Need

Active Washington, DC Registered Nurse license required
Valid driver's license and reliable transportation
Strong clinical judgment, documentation, and communication skills
Home health experience preferred
Acute care and wound care experience are a plus
