Rate: $30 per hour Eligible for a $100 scrub uniform allowance. Education And Experience/Qualifications Graduation from an accredited licensed practical nurse program required. Associates in a related field preferred. Must have 2 years of clinical experience providing nursing services to patients from diverse economic and cultural backgrounds, as well as exceptional communication, interpersonal and organizational skills. Outpatient primary care settings experience preferred. Bilingual English/Spanish is a plus. Contributes to a compassionate and caring environment for patients and families through kindness and active listening. Provides direct care to patients, including obtaining medical histories, vital signs, blood draws, preparative care for diagnostic testing, wound care, dressings, injections, and assisting the medical provider in the examination room. Demonstrates effective teamwork by interacting positively with colleagues, visitors, patients, caregivers, and families. Initiates open communication, expresses positive intent, and offers assistance to support patients, families, and caregivers. Maintains a safe and secure environment by following established procedures and protocols. Respects patient and family cultural, spiritual, and personal norms. Provides chronic care disease support by phone, in person, and in all aspects of care, treatment, and services, developing patient‑focused self‑management goals and care plans. Ensures gaps in care for screenings, chronic care tests, health literacy assessments, tobacco screenings, BMI, etc., are completed in established timelines. Attends and contributes to weekly multidisciplinary team meetings and daily huddles. Demonstrates strong nursing knowledge and skill in job performance. Coordinates care in collaboration with medical providers, ensuring continuity of care. Identifies patient care issues, provides information, recommends options, and implements medical providers’ directives. Serves as liaison to medical providers. Adheres to infection‑control policies and protocols to protect patients and employees. Documents timely and accurately patient care services in the electronic health record, tracking referrals, labs, and diagnostics. Passes a respirator fit test at time of hire and annually. Participates in regular staff meetings focused on coordinating care within an interdisciplinary team. Continues to develop knowledge and understanding of the history, traditions, values, family systems, and artistic expression of served groups, using appropriate methodological approaches, skills, and techniques that reflect cultural understanding.
