Community Transition Coordinator - IDD & Mental Health
MHMR of Tarrant County
MHMR of Tarrant County is seeking a Direct Care - Clinical Therapist/Social Work/Case Worker to provide intensive support in community settings for individuals with IDD and related mental health needs. The role emphasizes transitioning clients from programs to community-based services with person-centered planning. Qualified candidates have a Bachelor’s or advanced degree in social or human services and at least two years of service coordination experience; QIDP qualification is preferred, with #J-18808-Ljbffr MHMR of Tarrant County
