Interim HealthCare is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our growing team in Keene, New Hampshire. Position Details Location: Keene, NH Schedule: Monday-Friday | 8:30 AM - 5:00 PM Employment Type: Full-Time What You’ll Do As an RN Case Manager, you will coordinate and oversee the care of a dedicated patient caseload while providing skilled nursing services in the home. Responsibilities include: Conduct comprehensive patient assessments and develop individualized plans of care. Provide skilled nursing visits and case management services. Coordinate care with physicians, therapists, caregivers, and other healthcare professionals. Monitor patient progress and adjust care plans as needed. Provide patient and family education to promote independence and improved health outcomes. Complete timely and accurate clinical documentation. Supervise and collaborate with interdisciplinary team members to ensure exceptional patient care. Advocate for patients while promoting quality, safety, and continuity of care.
