Benefits include: New pay rates and sign-on bonus Work-life balance in a non-profit organization Comprehensive benefits: employer-matched 403B, tuition reimbursement, health benefits, eye coverage, PTO, mileage reimbursement, etc. Geographic-based scheduling Manageable patient caseloads No mandated call; dedicated RN staff handle this Flex day off during the week when working weekends with rotation and differential Daytime hours, Monday through Friday, with flexible visit times Responsibilities Assess/reassess patients and develop personalized care plans Collaborate with providers and communicate changes promptly Manage payment sources and ensure ongoing authorizations Educate patients and caregivers; initiate discharge planning on admission Complete documentation within 24 hours, including changes in condition and follow-up Evaluate patient progress and act promptly Oversee LPNs and Home Health Aides Participate in case management meetings and document accordingly Schedule patients independently per care plan Attend staff meetings and mandatory trainings Follow policies and ensure accurate OASIS documentation Qualifications Education: Nursing Diploma or Associates Degree in Nursing; Bachelor's preferred Experience: 2 years as a Registered Nurse, home health experience preferred Licensure: Valid Connecticut RN license or eligibility Skills: Excellent clinical, verbal, written, and organizational skills; ability to work independently; strong team collaboration skills Physical Demands: Ability to perform physical tasks including lifting and extensive standing Additional Information Coverage area: Guilford and Branford; rotating weekends and holidays #J-18808-Ljbffr
