Job Summary

Cuready Healthcare Staffing is seeking a qualified Registered Nurse, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Perform case management assessments for assigned patients.
• Develop and implement comprehensive discharge plans.
• Collaborate with interdisciplinary teams to ensure seamless patient transitions.
• Maintain accurate electronic medical records using Meditech and Midas systems.
• Adjust care plans as needed for dynamic, fast-paced environments.
• Work closely with physicians and mid-levels on cardiac and medical step-down units.

Qualifications

• Minimum 1-year current acute care case management RN experience.
• Active RN or Licensed Social Worker credentials.
• BLS certification.
• Active Kansas licensure.
• Experience with Midas and HCA case management preferred.
• Ability to work effectively in a busy, fast-paced environment.
