Licensed Practical Nurse (LPN) – Home Health

Potential Contract Opportunity

Schedule: Monday–Friday, 8:00 AM–5:00 PM
On-Call Requirement: 1 week per month (light/easy on-call rotation)
Hours: 40 hours not guaranteed
Mileage Reimbursement: $0.45/mile

Coverage Area: Peninsula Region – Williamsburg to Hampton, VA

Position Overview

We are seeking an experienced Home Health LPN to provide high-quality patient care and care coordination services throughout the Peninsula region. The ideal candidate will have a strong home health background, experience using HomeCare HomeBase (HCHB) , and a passion for helping patients achieve positive health outcomes in the comfort of their homes.

Candidates must have at least two years of recent Home Health experience and recent experience using HomeCare HomeBase (HCHB).

Key Responsibilities

• Conduct comprehensive assessments of high-risk patients
• Develop, implement, and reassess cost-effective plans of care
• Coordinate care transitions and collaborate with interdisciplinary healthcare teams
• Identify and arrange non-medical support services, including transportation and housing resources, to improve treatment compliance
• Monitor patient progress and intervene proactively to prevent adverse outcomes
• Serve as a patient advocate while maintaining privacy and confidentiality standards
• Maintain detailed and accurate clinical documentation in HomeCare HomeBase (HCHB)
• Coordinate specialty resources and community services as needed
• Promote health education, wellness initiatives, and illness prevention strategies
• Provide wound care and other skilled nursing services within scope of practice

Required Qualifications

• Current unrestricted Multi-State LPN License
• Minimum of 2 years of recent Home Health experience
• Recent HomeCare HomeBase (HCHB) experience required
• Current CPR/BLS Certification
• Current TB Test/Clearance
• Successful completion of wound care and competency testing
• Strong clinical assessment and critical thinking skills
• Ability to analyze complex medical information and make sound clinical decisions
• Reliable transportation and ability to travel throughout the assigned territory

• Ability to work independently while collaborating with an interdisciplinary team
