Social Worker- $4,000 Sign-On Bonus (to be paid throughout the first year of employment). Full-Time, Daylight shift; 8:00 a.m. to 4:30 p.m. Social Worker rates start at $21.40 per hour. The social worker is a part of the interdisciplinary team, responsible for identifying and developing care plans that address the psycho-social needs of the resident/patient. For assigned residents/patients, insure appropriate care plans, act as resident/patient advocate, staff education as appropriate. Develop goals for residents/patients and work with residents/patients to achieve these goals. Acts as liaison and provides educational support to residents/patients and their families. Qualifications Social Worker qualifications include: Current License under the Social Workers, Marriage and Family therapists and Professional Counselors Act, or a Bachelors, masters or doctoral degree from an accredited school or program in social work or social welfare. 1 year experience in health or related field. Strong written and verbal communications skills. Working knowledge of computers, Microsoft Word and Excel required.
